Overview of the role:
To help individuals to live an independent life in line with their care and support plan. Your role is to ensure that the people who use the HomeCareDirect service are supported to achieve their goals and aspirations to care standards whilst making a success out of directing their own support at home.
You will undertake care and support plans, annual support reviews, personal assistant inductions, training supervisions and other visits as requested by the individual or family or deemed necessary by HomeCareDirect. You also will provide personal assistant training in line with your competencies.
You will liaise with the HomeCareDirect Independent Living Team, families and care professionals to ensure that your visits and subsequent paperwork is completed in an efficient and timely manner so that the support packages you are involved in run smoothly and compliant with CQC standards.
You are required to own, or have access to car transport and be willing to travel as part of your role.
Starting salary of £15 per hour (equate to £29.250 per hour full time equivalent), increasing to £16 per hour after 6 months (equates to £31,200 full time equivalent).
You will be qualified as a Nurse Band 5 or above with a current NMC registration.
Experience in the domiciliary care industry is an advantage.
Level 5 Diploma in Leadership for Health and Social Care is desirable
Have a commitment to undertake continuing professional development (CPD).
Fully conversant with Microsoft Office Word, Excel, Power Point, Desk Top publisher, Outlook (Essential)
PTTLS (Preparing to Teach in the Lifelong Learning Sector) qualification desirable
Effective care-planning experience is essential to ensure that the highest level quality of service is delivered.
